REQUIREMENTS FOR REGISTRATION:-
- 4 Passport size Photos of Competent person.
- Copy of Certificates of Competent person.
- Appointment letter of competent person.
- Experience Certificate of competent person.
- Proof of Address of partners/ directors and competent person.
- Pan card of partners/ directors and competent person.
- Key Plan.
- Site Plan.
- Electricity Bill of premise.
- Rent Deed + Rent Receipt if rented.
- Proof of ownership viz. property papers or municipal taxes deposition slip.
- Proof of Constitution of the firm – Partnership deed/ MOA, AOA, etc.
- Letter Head – 10.
- Space and height should be minimum of 120 Sq Ft. and 9.5 Ft.
- Original copy of Drug License in case of change

- Proof of ownership viz. property papers or municipal taxes deposition slip.

DOCUMENTS ARRANGE BY US:-
- Application forms in Form 19.
- Challan of Fee Deposited.
- Declaration by the proprietor/ partners/ directors.
- Affidavit of Non Conviction – Firm/company/Proprietor/Partners/directors and competent person.
- Affidavit by partner/ proprietor/ director.
- Bio-data of Competent person.
- Affidavit of Competent person.
